Insurance Information and Access to DEBORAH® Services

Insurance Information
If you have health insurance we will need your insurance card and the insurance forms supplied by your employer for the insurance company. You will be asked to assign benefits from the insurance company directly to the hospital and the physician group that will treat you here.

The Center submits bills to your insurance company and will do everything possible to expedite your claim. Please remember that your policy is a contract between you and your insurance company, you may be asked to assist in obtaining payment by the Patient Accounting Department.

Bills for services provided at the Center will be submitted to your insurance carrier. These services are billed under the name, Professional Service Fund, and include all physician specialties.

Insurance Requirements
Your plan may have special requirements, such as a second opinion or pre-certification for tests or procedures. It is your responsibility to make sure the requirements of your plan have been met, if not, your admission may be delayed. Patients are responsible for initiating and obtaining a referral to a specialist if required by the HMO. A referral must be obtained from your primary care physician if you are covered by an HMO, PPO or other managed care plan. If You Don't Have Insurance
A representative of Patient Access Services will discuss arrangements with you. You may be requested to supply specific personal financial documents. This will allow the Center to be reimbursed, in whole/in part, for the services provided to you through state funding for uncompensated care.

Access to DEBORAH® Services

Physician Referral
Physician Referral is the easiest way to get access to any of DEBORAH’s services. Your physician makes one telephone call to DEBORAH’s Physician Referral Line at 1-800-214-3452. DEBORAH does the rest of the work.

Appointments are made promptly and at your convenience. Physician referrals must be obtained by anyone using a managed care network such as an HMO or PPO.

No Physician Referral
In some cases it is not possible to get a referral from a private physician. If you have a cardiac, pulmonary or vascular history, an appointment can be made without a physician referral.

Transfer from Another Hospital
Patients admitted to another hospital for an emergency can be transferred to DEBORAH by their attending physician. One telephone call to the Physician Referral Line at 1-800-214-3452 will start the transfer process. Transfers can be made 24 hours a day, seven days a week. Most of the time transfers can be accommodated immediately.

Facts You Should Know
There are no limitations of services with the exception of heart transplants. We perform a complete range of heart, lung and vascular related tests and procedures available. Services also include second opinions, appointments with specialists, and diagnostic tests. DEBORAH accepts most insurance plans including Medicare, Medicaid, Horizon/Blue Cross, and participates in over 50 managed care networks including most major HMO companies.
